Our organisation

Our purpose

EMPHN exists to build better health outcomes and better health experiences for communities within eastern and north-eastern Melbourne through a more integrated healthcare system.

Working alongside our partners, we facilitate system improvement across the catchment by addressing health inequities, strengthening primary care, enabling integration and supporting sustainable health system improvements.

Our role as a Primary Health Network

Established by the Australian Government, Primary Health Networks exist to improve the efficiency and effectiveness of healthcare services, reduce fragmentation and improve health outcomes.

EMPHN delivers this role through three key functions:

Coordination

EMPHN connects health professionals, hospitals, community services, governments and consumers to improve integration across the healthcare system. By strengthening relationships and collaboration, we help ensure people experience more connected, coordinated care.

Commissioning

EMPHN uses data, evidence and community insights to identify unmet needs and gaps in care. Working with partners, clinicians and people with lived experience, we commission and co-design services that improve access, quality and outcomes for the community.

Capability building

EMPHN supports general practice and the broader primary care sector through education, quality improvement, data and digital health initiatives, helping providers deliver high-quality, person-centred and future-ready care.

Our strategy

Over the past year, EMPHN has continued to progress the priorities set out in our Strategic Plan, focusing on improving health outcomes, reducing inequities and strengthening primary healthcare across the region.

Through targeted investment, partnership and innovation, EMPHN is working towards a more connected and equitable health system where people can access the care they need, when and where they need it.

Strategic pillars

1.

Drive equitable access and outcomes for communities

2.

Advisory groups

EMPHN values the knowledge, perspectives, and backgrounds of people involved in both providing and using primary healthcare services within the eastern and north-eastern Melbourne catchment.

EMPHN works closely with consumers, community members, clinicians and Aboriginal stakeholders to help guide our work. The following groups play an important role in providing advice, feedback and insights that strengthen our decision-making and support better outcomes for the communities.

Community and Consumer Council

Our Community and Consumer Council is an active group of people with diverse experience, who provide subject matter expertise, insights and advice to support our organisation’s strategy and the way in which we design our services.

Clinical and Practice Council

Our Clinical and Practice Council is comprised of clinicians and health professionals across the full spectrum of health care. This includes general practice, pharmacy, nursing and allied health. The Clinical and Practice Council plays a pivotal role in advising and assisting us to effectively listen to and respond to our community’s priorities and concerns.

Aboriginal Consultative Council (ACC)

Our Aboriginal Consultative Council aims to build trust and strengthen shared decision-making to support self-determination within the catchment. The Council, consisting of Aboriginal Community Controlled Health Organisations (ACCHOs) and Aboriginal Community Controlled Organisations (ACCOs), offers insights and experiences across all levels of strategy, operations, planning, policy development, and strategic advice.

WiseCrowd

Our consumer and community panel is made up of people with a personal or professional interest in improving healthcare in our region. Members can have a say on the future of healthcare in the catchment.

By sharing their health care experiences with us, members can:

  • Help us improve health services
  • Engage in online activities like surveys, virtual discussions, and social media campaigns, join in-person or online workshops or get involved in committees and working groups.
