Building the future
Overview
Foundations for the future

During the year, EMPHN designed and built the foundations for one of the most significant transformations in its history through EMPHN OneCare. This innovative model brings together funding streams, service pathways and care coordination functions to create a more integrated and person-centred system of care.
Designed to provide a more connected experience for consumers and providers, EMPHN OneCare focuses on continuity, coordination and support. It will help people navigate complex health and social care needs while enabling services to work together more effectively. The model represents an important shift from fragmented service delivery towards a system that is easier for people to access and better equipped to respond to their needs.
Alongside EMPHN OneCare, we are building a contemporary approach to supporting general practice and the broader primary care sector. Moving beyond traditional activity-based support, the new model will deliver more coordinated, evidence-informed and outcomes-focused programs that strengthen capability, improve consistency and reduce duplication.
Its development is being informed by clinicians, consumers and health system partners, ensuring it responds to the needs of those who deliver and use primary care.
Together, these new ways of working are laying the foundations for a more connected, sustainable and responsive primary health system. By bringing together commissioning, integration, capability building, stakeholder engagement and data-driven improvement, EMPHN is creating the conditions for lasting change and better health outcomes across our communities.
As these initiatives move from design into delivery, they provide a strong platform for the future and reinforce EMPHN’s role as a trusted partner, system leader and catalyst for primary health reform.
The strategic pillars
EMPHN's future is guided by three strategic pillars that will shape how EMPHN responds to the evolving needs of our communities and health system.
Together, these priorities focus on improving equity, strengthening the use of data and insights, and enabling greater integration across care.
